Hope for Healing

Project Title: The Mobile Outreach Project DuPage County

Award Amount: $32,000

Summary: Hope for Healing will partner with Live4Lali and Inspiration Outreach to implement a harm reduction mobile outreach unit. The project includes outreach and engagement of individuals in need of support, linkage to recovery-oriented services and HIV, Hepatitis C Virus (HCV), and sexually transmitted infection (STI) testing and treatment, Medication Assisted Treatment (MAT) and other primary and specialty care services. The mobile unit will provide naloxone, drug safety testing supplies, other clean/safe equipment, tangible safety-based education, safe sex supplies, food, toiletries, water, peer-driven Cognitive Behavioral Therapy (CBT)/ Rational Emotive Behavior Therapy (REBT) and motivational interviewing, case management, and linkage to ancillary services.

Advocate Health and Hospitals Corporation dba Advocate Good Samaritan Hospital

Project Title: Patient Engagement and Retention in Specialist Initiated Assessment and Treatment (PERSIST) Pathway to Community Recovery

Award Amount: $25,000

Summary: Advocate Good Samaritan Hospital will expand their Community Linkage Specialist support to patients struggling with Substance Use Disorder (SUD)/Opioid Use Disorder (OUD) to include additional hours to engage new community providers that assist with housing/sober living, MAT, and unmet social needs and provide individualized information and referral including transportation assistance to and from appointments.

NAMI DuPage

Award Amount: $31,000

Summary: NAMI DuPage will provide a twice per month support group co-facilitated by a clinician experienced with substance use disorders and a peer with co-occurring conditions and peer counseling services to clients with a dual diagnosis.

McDermott Center dba Haymarket Center

Project Title: Together DuPage

Award Amount: $37,000

Summary: A project-dedicated recovery coach/Certified Alcohol and Other Drug Counselor (CADC) will work with the DuPage County Jail to engage recently released and soon-to-be-released inmates in recovery supports via the evidence-based recovery coaching model.
